With the information you provide, our staff will enter your information in the appropriate fields on each form, send them to you via email to sign, e-file the documents with the appropriate Clerk of Courts office, request a hearing and provide you with an exclusive free gift while supplies last. Please note, you have the option to request up to 3 additional forms be completed on behalf of the respondent, request a summons for service in the city the respondent resides in OR request a notice of action; choosing 2 out of 3 options will result in an additional fee of $150.00.
